1) Complete the Request for Policy Loan form.

2) Sign and date the form.

3) If an irrevocable beneficiary or assignee has been named, that individual must consent, in writing, to the policy loan by signing the consent section of the form.

4) Return the signed and completed form to our office. In order to process this change, we require proof of your signature (a copy of your Driver's License, Medicare Card, etc.)
