1) The owner completes the Beneficiary Change Form. Insureds who are not policy or certificate owners may not complete this form.

2) List the new primary and contingent (secondary) beneficiary(ies) even if only one or the other is being changed. This ensures that your records will be accurate and the proper person(s) will be paid.

3) Clearly designate the beneficiary(ies). This identifies the proper recipient(s) so that there is no doubt who the beneficiary(ies) are, and in what shares.

4) Fill out the form completely and accurately. Give full names, each beneficiary's address, date of beneficiary's birth (this is mandatory if a minor) and the beneficiary's relationship to the insured.

5) Sign and date the form.

6) If an irrevocable beneficiary has been named previously, that individual must consent, in writing, to the change by signing the consent section of the form.

7) Return the completed form to our office. In order to process this change, we require proof of your signature (a copy of your Driver's License, Medicare Card, etc.)

8) We will process your request when we receive the signed document and we will return an acknowledged copy of this form for your records.
